     One person per form filled out and submitted; please use a separate form for each family member volunteering their time and talents.      
     All adult volunteers must fill out a Background Check to volunteer and  attend a "Protecting God's Children Workshop".    You may sign up for a workshop at www.aod.org.  All High School age volunteers are required to attend " Called to Serve".   Please call our parish office, to find out more information (248-879-1310).

If you wish to be removed from one of the current ministries you are participating in, please place the letter “D” next to that ministry if using a paper copy.  Otherwise, please contact the Parish Office at 248-879-1310.
